Student accident insurance will again be available this year to all students. IMPORTANT: To find more information about the student accident insurance, parents should go to our homepage - elkhornvalleyschools.org - click on the Menu and then click on Links. Locate Student Accident Insurance and click on that. This link will take parents directly to the Student Assurance Insurance Company website where parents can locate information and the application form. Parents will be required to complete the application and purchase the insurance online. The application will be submitted directly to the insurance provider. High school students, grades 9-12, who will be participating in fall activities will be required to have their application form completed and submitted to the company OR have a signed waiver form completed and turned into their coach within the first two days of practice.
Waiver forms were made available at the mandatory parent meeting held on August 1. If you need a copy of that waiver form, please contact the school or your child's coach. Thank you!!
